This is the source code for the template for the HTML version of the first email message sent that announced this survey.

As you can see, style sheet information for formatting the message is embedded in the message. But links to graphics, such as the agency logo, are used and will pull the graphic file from the questionnaire web site when the message is rendered in the respondent's email reader. The <BASE> tag is set automatically to your web site's URL when the messages are created, so the respondent's email reader will look for all the linked files there.

<p>Dear {$q_name}</p>

<p>We have now activated GAO's survey regarding your project's
participation in the recreational fee demonstration program. To obtain
valid data, we need a response from all the projects participating in the
program including yours. In some cases, your project may involve multiple
sites however we need one consolidated response for the project as a
whole, which incorporates all sites participating in the project.</p>

<p>This email is being sent to you because, according to our records,
you are the coordinator for the <b>{$PROJECT}</b>
project.</p>

<p>If you are a manager for more than one fee demonstration project, you
should have received a separate e-mail for each project and will need to
complete a separate questionnaire for each project. (Note: You will be
sent a separate user name and password for each questionnaire.) If you
have further questions or need assistance regarding the questionnaire,
please contact either Brian Estes or Frank Kovalak of GAO as listed below. </p>

<p>Please note that the scope of this survey is limited to fees and fee
collection methods whose revenue is counted under the fee
demonstration program. </p>

<h2>About the Survey</h2>

<p>At the request of the U.S. Congress, the U.S. General
Accounting Office (GAO) is conducting a survey of all projects
participating in the recreational fee demonstration program. The GAO is
an independent, non-partisan agency that provides information to the
U.S. Congress. The survey includes approximately 380 projects among the
4 agencies participating in the fee demonstration program: the Bureau of
Land Management, the National Park Service, the U.S. Fish and Wildlife
Service and the U.S. Forest Service. The survey covers the types of
recreation fees charged, fee collection methods, and coordination efforts
with adjacent recreation areas.</p>

<p>We will present the results of the survey in a report to the Congress by
September 2001.</p>

<p>Once the report has been issued, you will be able to access it through
GAO's Homepage: www.gao.gov. You can also order a printed copy
from GAO through our Homepage or by calling GAO. When the report is
issued, we will provide information on how to obtain your copy. </p>
